Unrecognised Free Water Loss in a Patient with Lithium- Induced Nephrogenic Diabetes Insipidus Can Cause Acute Kidney Injury and Near Fatal Hypernatremia..

چکیده

Acquired nephrogenic diabetes insipidus is most frequently caused by lithium which is used to treat bipolar disorder. The reported incidence of acquired NDI in patients on long term lithium treatment is as high as 55% [2]. Lithium-induced NDI results from inactivation of adenylyl cyclase and thus the phosphorylation of cytoplasmic urinary aquaporin 2 AQP2 by protein kinase A, and consequently ADH-induced translocation of cytoplasmic urinary aquaporin 2 (AQP2) to the apical membrane [3]. Lithium also down regulates the protein abundances of urea transporter, UTA1, UT-B, thus reducing medullary interstitial osmolality [4-6]. Lithium induced NDI should be suspected in any patient on longterm lithium therapy who develops unexplained hyper natremia so that prompt treatment can be instituted.

Lithium-induced nephrogenic diabetes insipidus.

BACKGROUND Lithium can cause nephrogenic diabetes insipidus in up to 20 to 40 percent of patients currently taking the medication, and a subset of these patients will have a persistent concentrating defect long after lithium is discontinued. They are at risk for serious hypernatremia when fluid intake is restricted for any reason. METHODS MEDLINE as used to search the key words "nephrogenic d...

Lithium intoxication-induced acute parkinsonism complicated with hyperparathyroidism and nephrogenic diabetes insipidus: report of a case.

OBJECTIVE To describe a patient with lithium intoxication presenting as acute parkinsonism, adverse metabolic effects and nephrogenic diabetes insipidus (DI). CASE REPORT We report a case of a 67-year-old woman with a bipolar affective disorder who was treated with lithium for 10 years. Under concomitant renal insufficiency and urinary tract infarction, she experienced progressive hand tremor...

Lithium-associated primary hyperparathyroidism complicated by nephrogenic diabetes insipidus.

Lithium-associated hyperparathyroidism is the leading cause of hypercalcemia in lithium-treated patients. Lithium may lead to exacerbation of pre-existing primary hyperparathyroidism or cause an increased set-point of calcium for parathyroid hormone suppression, leading to parathyroid hyperplasia. Lithium may cause renal tubular concentration defects directly by the development of nephrogenic d...

Severe hypernatraemia due to nephrogenic diabetes insipidus - a life-threatening side effect of chronic lithium therapy.

Renal toxicity of long-term lithium therapy is a common problem. Nephrogenic diabetes insipidus is the most frequently encountered complication, but often remains unrecognised because of the rather benign symptoms. We present a patient with long-term lithium therapy who developed life-threatening hypernatraemia due to insufficient oral fluid intake after elective spinal surgery. Careful daily s...

Does lower urine-specific gravity predict decline in renal function and hypernatremia in older adults exposed to psychotropic medications? An exploratory analysis

BACKGROUND Exposure to psychotropic agents, including lithium, antipsychotics and antidepressants, has been associated with nephrogenic diabetes insipidus (NDI). This is especially concerning in older adults already at risk of developing chronic kidney disease (CKD) and hypernatremia with advanced aging. This study investigates whether commonly performed random urine-specific gravity (USG) test...
